Access One-on-One Support for Lifestyle Change, to Meet Health Goals

For many people one of the hardest parts about trying to improve your health or overcome a health condition is finding a plan that works and staying accountable. When progress is slow or the plan isn’t going as expected, it’s easy to feel discouraged. And knowing when to keep going and when to pivot to prevent derailing all together isn’t always clear.

That’s where having some one-on-one support can make all the difference.

As a SchoolCare/Cigna participant, you have access to coaching support and care coordination in a variety of ways, all at no out-of-pocket cost.

Get support for pre-diabetes, type 2 diabetes and hypertension with Omada

Did you know 1 in 3 Americans have pre-diabetes, and 80% don’t know it? Without intervention, pre-diabetes can lead to type 2 diabetes, increasing the risk of heart disease, stroke, and other complications. Early detection and lifestyle changes are key to reducing these risks.

Omada For Prevention offers SchoolCare participants ages 18 and older a personalized plan to improve diet, exercise, stress management, and sleep. The program includes one-on-one coaching, free tools like the Omada app and a connected scale, and incentives through Good For You! Well-Being Program ($100 for completing 16 weeks of the program and $250 for meeting a weight loss goal of 5%). 

Omada Complete, introduced last year, added programs for participants already diagnosed with type 2 diabetes and/or hypertension. These programs include the same tools as the prevention program, with additional monitoring options like a continuous glucose monitor, glucose monitoring test strips, a blood pressure cuff, and access to a clinical specialist for guidance, based on the participant’s needs. Create Healthy Habits, Plan Lifestyle Changes with Coaching and Case Management

Cigna Healthcare Advocates are available to all participants for support, from one-on-one health coaching to case management for complex medical conditions. The team, made up of clinical experts, doctors, and pharmacists, provides personalized guidance based on your health goals or needs. If you receive a call from Cigna, it’s likely an Advocate offering help at no extra cost to you.

Advocates assist with understanding medical conditions, exploring treatment options, coordinating follow-up care, and developing healthier habits. They provide encouragement, help you stay on track with your health goals, and support managing chronic or complex conditions.

Earn Good For You! Well-Being Program incentives:

  • Engage with a Telephonic Health Coach for support in making progress toward overcoming chronic conditions ($100) and making progress toward ($25) and achieving ($50) health goals. Each incentive can be earned up to two times per year. View the health coaching flyer here. 
  • When you work with a Case Manager on complex medical conditions, earn $100 for setting up care coordination, and $250 for progress toward a goal, as determined by your Case Manager.

To earn your incentives you must complete your annual online health assessment found in your myCigna portal.  Login to myCigna and click on “My health assessment” under the wellness tab to get started. It takes just 10-15 minutes. You must complete it by May 30, 2025, to earn incentives for this plan year.

Overcome Pain with Hinge Health

Musculoskeletal pain (back, muscle, joint) is a leading cause of claims among SchoolCare participants. Whether you're dealing with chronic back/joint pain or recovering from an injury, Hinge Health offers a virtual physical therapy program at no extra cost.

Hinge Health sends participants a tablet loaded with its app, and wearable sensors to track exercises, and assigns each participant a one-on-one coach to help guide a personalized plan tailored to your needs. Get back to moving and doing what you love with Hinge Health.

New for 2025: Fill out only one entry form when you watch the Pop Quiz video before you enter the raffle and include your answer on your raffle entry form. If you are short on time and wish to enter the raffle now and answer the pop quiz later, you can, you will just need to fill out a separate Pop Quiz entry form. One raffle entry and one pop quiz entry per person, per week.

Cash incentives, payments, and rewards are available upon completion of the Health Assessment annually. Amount paid to well-being program participants is taxable income. Please review with your tax consultant for more information.
